523 year old white female desiring more youthful look
This is a 53 year old white female who wanted a more youthful appearance. Her concerns included blotchy pigmentation, nasolabial folds, and her neck. Her goals were to look a little less tired and to get rid of the "flappy neck" Exam: The patient had prominent nasolabial folds, hollow appearing cheeks, jowls, lax skin, and a moderate thickness fat pad under the jaw. Additional findings included platysmal bands in the anterior neck and a somewhat forward trachea. Surgical technique: A lower facelift was performed using a smas-platysma lift. The cheek fat pad (of Bichet) was elevated and to reduce the nasolabial folds and correct the jowls. Using a submental incision (under the chin), the redundant platysma muscle was reduced, and sutured along the midline. A notch was made in the muscle to allow the muscle to rise up under the jaw and give a deeper jawline from the profile view. The jawline was blunted somewhat by the forwardly located trachea, and by the patient's post-op weight gain (~15 lbs). The patient is approximately 14 months post-op. More
